Out Of area Health Benefits - Call in starts July 1stIt will soon be time to call in for your out of area benefits. You must call CHS starting at 8 AM on July 1, 2009. The sooner you call, the better. The Phone numbers are 800 628 5720 or 541 444 1236. One of the following benefits is available: $400 Vision Plus $500 maximum Pharmacy thru Pequot Rx, which you may use throughout the calender year 2009. Future call-in periods will start on October 1, 2009 and January 4th 2010.
